Question: Sample grievance letter to the methadone clinic?

I got to the window and realized that i didnt have my weekend bottles with me and had time to go home and get them but they were suspended for one month. during this time i am eligible for another bottle but since i simply forgot to bring the bottles now i have to wait for three months before i can get my next bottle, i see the psych doc for depression and forgetfulness is a sympton of depression i need to know how i can resolve this matter with a sample grievance letter |
